REGISTRATION

To register, please complete the following:

• Registration for each child. (PDF )

• Application for each child. (PDF)

• Pay $100 registration fee for school and/or $100 registration fee for childcare, latchkey or summer program for each child. Checks may be made payable to “Our Lady of Bethlehem.”

• For any new students, an original birth or baptismal certificate must be presented to the office at the time of registration.
